About The OpportunityWe have an excellent opportunity for a driven professional to join the Integrated Chronic Care for Aboriginal People Program (ICCAPP) team, as the Clinical Nurse Specialist, Level 2 (CNC2). Newcastle ICCAPP is co-located with the Referral and Intake, Planned Care for Better Health and Out of Hospital Care teams. ICCAPP is operationally managed by a Nurse Manager.
Operating hours: 8am to 4.30 pm Monday to Friday, closed weekends and public holidays.
The CNC role works in conjunction with the Registered Nurses on the team. There is strong collaboration with internal and external stakeholders (clinical and non-clinical) to provide comprehensive, patient centred, case management, support at Medical clinics based at local Aboriginal Medical Centres, health screening, health related education and support. You will be provided with appropriate clinical and cultural support to ensure the capacity to develop professionally and personally. ICCAPP has teams across the District, who you will liaise and collaborate with to provide clinical support and guidance.
About YouThe ICCAPP team is seeking a dedicated individual who is passionate about providing holistic, culturally appropriate health services for Aboriginal people and their families.
- Excellent communication skills with a strong focus on patient care.
- Exceptional work ethic and strong time management skills.
- Ability to work effectively with Aboriginal people within their community to provide holistic care.
- Confidence to work independently within your scope of practice while knowing when to seek support.
- Experience or strong interest in diabetes and/or cardiology health, with a deep understanding of chronic disease management & patient care strategies.
- Current Authority to Practice with AHPRA who has at least 5 years full time equivalent post registration experience.
- Post graduate qualification in chronic disease or evidence of working towards same and at least five years post graduate nursing experience with 3 years' experience in chronic disease management.
- Current NSW Driver's License and capacity to travel for work.
