About the Role:

We are seeking a Registered Nurse who is passionate about supporting clients with chronic conditions through culturally safe, holistic care coordination.

The role of the Chronic Disease Nurse is to support clients with chronic health conditions through culturally safe, coordinated care. Working closely with GPs, Practice Nurses, and the wider health team, you'll assist with chronic disease management plans, health checks and referrals to allied health and community programs.

You'll help coordinate care across internal teams and external providers, support medication management, and contribute to case management for complex clients in partnership with the Chronic Disease and Outreach teams.

The role also includes supporting preventative health activities designed to promote community wellness, such as immunisations, health promotion, education, wound care, and assistance with minor procedures. You may also be involved in case conferencing, infection control, and mentoring Aboriginal Health Workers or students as required.

To be considered for this position, you will need:

  • Current APHRA registration as a Registered Nurse (Division 1)
  • Experience working in chronic disease management, developing and managing chronic disease care plans and running effective chronic disease clinics
  • Demonstrated experience in delivering preventative health care, health promotion, clinical support, and minor procedures
  • Understanding of the health issues impacting Aboriginal and Torres Strait Islander communities
  • Confidence using Microsoft Office and electronic client management systems
  • The ability to work autonomously and take initiative, while contributing to a collaborative, multidisciplinary team
  • A current Working with Children Check
  • A satisfactory National Police Check
  • A current Driver's Licence
