The Opportunity

The Premiers Youth Initiative (PYI) aims to prevent homelessness among young people leaving care by diverting them from the homelessness service system before they become homeless or at risk. The role of Transitional Support Worker will be to provide transitional support to young people in targeted locations to build their capacity to establish and maintain long-term accommodation, with a particular emphasis on building capacity to establish and maintain shared accommodation over time through up to 4 years of transitional support.

The Essentials

  • Tertiary Certificate in Youth Work, Community Welfare, or Social Work
  • Experience working from a trauma-informed, strengths-based approach with young people who have lived in Out of Home Care
  • Experience in incidental counselling and mentoring of young people with complex needs
  • Ability to build relationships, network, work cooperatively with, refer to, and advocate for diverse support services
  • Knowledge of cultural diversity and community needs related to child and young person care, and managing reports of risk of harm
  • Skills in goal setting and action planning for young people leaving Out of Home Care
  • Understanding of rights, legislation, and resources for children and young people in Out of Home Care, including leaving and aftercare planning
  • Ability to maintain confidentiality, prioritize demands, meet deadlines, and work independently
  • Strong planning, organizing, investigative, assessment, problem-solving, and analytical skills

  • Criminal History Check
  • First aid & CPR certification
  • Pre-employment functional medical
