
Support Coordinator

Aligned Horizons' is seeking a talented and dedicated Support Coordinator to join our team in Melbourne, VIC 3000. As a casual Support Coordinator, you will play a crucial role in assisting older adults and individuals with disabilities to access the support and services they need to live independently and with dignity.
What you'll be doing
Collaborate with clients to develop personalised support plans that address their unique needs and goals
Coordinate and facilitate the delivery of a range of support services, including in-home care, allied health, assistive technology, and community engagement
Advocate on behalf of clients to ensure they receive the appropriate level of support from government and community organisations
Monitor client progress and adjust support plans as needed to ensure ongoing satisfaction and positive outcomes
Maintain comprehensive case notes and documentation in accordance with organisational and regulatory requirements
Engage in regular communication with clients, their families, and other stakeholders to ensure seamless service delivery
What we're looking for
Relevant tertiary qualification in social work, occupational therapy, or a related field
Minimum 3 years' experience in a similar support coordination or case management role
Demonstrated understanding of the aged care and disability support sectors, including relevant legislation and funding models
Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse clients
Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for clients
Proficiency in using client management software and other relevant technology
Commitment to the principles of person-centred care and a strength-based approach
