About the RoleThe Family Support Practitioner will work with families in partnership to empower and build their agency to make effective change and overcome their identified family challenges. The role is heavily outreach based, engaging families in their homes, and translating initial engagement into a strong ongoing partnership. Once engaged, the role will not identify the family issues in a practitioner-led assessment, but will deliver interventions that build family awareness and insight into their history and current family challenges so that families self-identify their key challenges and lead their own change.
Responsibilities- Meet individual case performance targets as per weekly, monthly and quarterly dashboards.
- Provide high quality and intent-based practice in all interactions with clients, aligned with ATSICHS values and vision.
- Provide high quality case management practices and record keeping that adhere to operational requirements.
- Engage clients into the service through a positive and warm outreach approach.
- Facilitate a client-led assessment and Care Plan to identify the client's goals, including the entire family, where appropriate.
- Provide role-specific targeted interventions and family-facilitated group meetings to achieve client goals.
- Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems and MobNet tasks.
- Cultivate productive working relationships at all levels to support internal and external collaboration, value differences and diversity, and guide staff development.
- Current Bachelor's Degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention or Criminology with 2 years of relevant experience in the social services field.
- OR: Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 3 years of experience in the social services field.
- With at least 1 year of experience in a case management role.
- Ability to work in a performance-driven environment to meet weekly, monthly and quarterly targets.
- Ability to build client agency and facilitate change empowerment while discouraging client dependence on supports.
- Knowledge of behaviour change interventions that build client self-awareness and active change toward identified goals.
- Experience working with the whole-of-family to meet total family needs; coordination of multiple services to provide holistic care.
- Knowledge of factors contributing to vulnerability, disengagement, and intergenerational trauma; experience with case management for high-risk or crisis clients with strong planning and organisation skills.
- Knowledge of supports available in mental health, substance misuse, education, employment and housing.
- Willingness to travel across the Logan and/or Brisbane region as per program location.
- Current driver's licence.
- Valid Police Check.
- Flexibility to work outside core 08:30am-5:00pm business hours to meet client needs.
- Working with Children's Card (Blue Card) or be eligible to apply.
- Permanent full-time position with starting annual salary of $93,613 (depends on qualifications and experience).
- 12% Superannuation + Not-for-Profit Salary Packaging (increases take-home pay).
- Work for a community-controlled organisation.
