About the facility
Our major sites include University Hospital Geelong in Geelong's CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the role
As the project officer will work within the Childrens and Families outpatient team to map and define all existing referral pathways and available services for paediatric patients as well as within the LHSN and design a centralised system for triage, flow, communication and support between the hospital network, referrers and families.
As the Project Officer you'll work closely with the steering committee to ensure that relevant data is collected and model developed to achieve project outcomes.
