The Social Worker / Psychologist position within the Assertive Case Management (ACM) program involves working as part of a multidisciplinary team to provide intensive care coordination for consumers with chronic and severe substance use issues.

The ACM program targets clients with complex drug and alcohol dependence who experience multiple needs. It aims to support stabilization, reintegration, and manage underlying cognitive impairments where relevant.

Program outcomes include reducing substance use, preventing hospital admissions related to substance issues, improving access to long-term treatment, enhancing physical and mental health, nutrition, social functioning, and reducing relapse risk through prevention strategies.

The successful candidate will act as a role model and promote a culture reflecting NSW Health CORE values: Collaboration, Openness, Respect, and Empowerment, through positive interactions with patients, clients, and colleagues.
