Our service : Universal Aftercare Services (UAS) is a low to high intensity non-clinical trauma informed support and transition service for people who have attempted suicide or are experiencing a suicidal crisis. The UAS program is built on personal connection and integration with community services. The aim is to assist those at risk of suicide to stay safe and better access high-quality support, while building the capacity to self-manage stress and improve mental wellbeing. On Offer :

  • Excellent training & career development Role Overview :
  • Provide direct and personalised assistance to people who have survived a suicide attempt or are experiencing a suicide crisis by providing referrals and linking with appropriate services, monitoring and reporting progress.
  • Use personal lived experience of mental health recovery to connect with people in a recovery-oriented, person-centred, and strengths-based way, and / or provide support with a demonstrated knowledge of skills that utilise non-clinical approaches to working with suicidality, trauma, emotional distress, grief and loss, and relationship breakdown.
  • Increase service participant's social connectedness and strengthen links to support networks, for example, families, friends, peers and carers, while simultaneously building distress tolerance and the capacity to alleviate and manage psychosocial distress and suicidal crisis.
  • Provision and documentation of mental health assessments, including completion of mental state examination, support planning, safety planning and mandatory reporting as required.
  • Consultation, liaison and linkage with internal and external service systems to provide a full range of tailored supports, including collaboration with Hospital and LHD Mental Health teams, and to improve the capacity and capabilities of other supporting workforces to support at risk individuals.
  • Provide regular, flexible support to consumers, and assist them to review their needs and resources and monitor activities identified in Individual Support Plans and Safety Plans.
  • Routinely perform Outcome Measures as per the Stride Outcome Measure Framework, and facilitate other reporting, feedback and research activities as required. Requirements :
  • Minimum of a Certificate IV in a Community Services related discipline (including Peer Work)
  • Hold, or be willing to work towards, a Certificate IV in Peer Work and / or complete Safe Story Telling training, or similar, as identified by Stride
  • Personal lived experience of mental health recovery and proactivity in managing wellbeing and / or experience providing psychosocial support with an ability to demonstrate understanding of mental health recovery and recovery-orientated practice.
  • Minimum 2 years' experience working with people at risk of suicide, and supporting people with mental health challenges
  • Must have Current Driver's License and willingness to travel often through Western QLD PHN region to see the participants of the program in a face-to-face capacity
  • Ability to think and act calmly and deal sensitively with distressed people
  • Demonstrate effective listening, communication skills, and empathy
  • Demonstrated problem solving and organisational skills
  • Ability to work autonomously, but also as part of a multi-disciplinary team
  • Understanding of the principles of duty of care, rights to privacy and confidentiality
  • Commitment to professional development and willingness to participate in training as required
  • Basic computer skills including record keeping, case note and data entry
  • Must be eligible to apply / currently hold a Blue Card
