Infinite Aged Care is seeking a dedicated Clinical Care Manager to join our team in Hahndorf, SA. Bring your dedication, clinical expertise, and mentoring skills to a team that values care and quality.

Responsibilities
  • Lead, guide, and mentor others to ensure the very best outcomes.
  • Build rapport with residents easily and take pride in their happiness.
  • Communicate effectively and think on your feet to meet the needs of residents and the facility.
  • Develop and maintain care plans in collaboration with residents and the care team.
  • Prepare and conduct clinical assessments.
Qualifications
  • Have a valid AHPRA Registration
  • Experience in preparing and conducting clinical assessments
  • Experience developing care plans
  • Familiar with AN-AAC and associated compliance requirements
  • Willing to work to meet the needs of the residents and facility
  • Hold valid NDIS Workers Screening Clearance (or ability to get one)
