Key Selection Criteria:

· Demonstrated ability to work professionally, both independently and within a team, to achieve consumer goals;

· Demonstrated commitment to provide professional Health Care Worker services that are person directed and safe, using a wellness and enablement approach;

· Ensure confidentiality is maintained at all times, and continues after this employment arrangement has ceased;

· Demonstrated ability to maintain accurate, subjective and timely documentation;

· Professional interpersonal, oral, written communication skills, including information technology skills;

· Demonstrated ability to provide person-focused care to the consumer.

· Demonstrated ability to interact with consumers so as to provide efficient and timely home care services.

· Demonstrated ability to work within a harmonious relationships with other team members so as to maximise outcomes that benefit both the consumer and HRH.

Certificate IV in ageing or individual support

Victorian Drivers Licence
