ConnectWell Community Health is seeking a Day Program Lead, 35 hrs/week, ongoing. This position provides a comprehensive benefit package, including HOOPP (Healthcare of Ontario Pension Plan).

Position Summary: You will be responsible for organizing and delivering day program supports for individuals with complex medical, physical and developmental needs as part of a team of support professionals. This role combines leadership, person centered/individualized planning and staff support to maintain high-quality service delivery and foster a collaborative team environment.

Qualifications:

  • Diploma or degree in one of the Social Sciences from a Community College or University or equivalent combination of education and experience
  • Substantial and proven experience working with individuals with complex medical, physical and developmental needs
  • Experience with adult day programs, an asset
  • Knowledge of programs and services that improve the quality of life for individuals with physical and/or intellectual disabilities
    Leadership experience, an asset

Reporting Relationship: The successful candidate will report to the Team Lead, Respite Day Programs.
