This position will deliver intensive, clinical assessments, interventions and support to people who are at risk of frequent relapses in their mental health and recovery, whom without intensive clinical interventions, would result in them requiring frequent hospitalisations to an acute inpatient service. This position will work in collaboration with other community clinicians to reduce emergency department presentations, readmissions and overall lengths of stay for the identified consumer cohort.

Salary
- Clinical Nurse Consultant NG7: $132,562 - $143,924 per annum
- Health Practitioner HP4: $128,169 - $137,918 per annum
Mandatory Requirements
- Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association within Australia. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
