Description : About The Role

As a key member of the ATSICHS Brisbane team, the Social Health Care Coordinator manages the intake system by screening and assessing the needs of families and individuals. This includes delivering case management and providing a range of assessed social and wellbeing supports to meet holistic needs. The Coordinator effectively manages and coordinates the service delivery model encompassing mental health, wellbeing, alcohol and other drugs, and social needs of clients.

Duties

1. Provide screening and assessments for clients referred to social health to determine the required support.
2. Deliver comprehensive assessments for clients identified for case management, develop goal-oriented case management plans, and deliver targeted supports as per the case plan.
3. Respond appropriately to critical incidents, providing wellbeing first aid to clients in distress.
4. Manage high-quality case management practices.
5. Ensure quality management through the use of LogiQC systems.
6. Coordinate with local medical clinics to facilitate integrated mental health and social services for patients.
7. Engage with community organizations and government agencies for information sharing, referrals, networking, and promoting ATSICHS Brisbane services.
8. Maintain self-management and contribute to team efforts.
9. Build productive relationships at all levels, fostering cooperation, trust, and respect, and supporting staff development.

Skills and Experiences :

Required :

• Bachelor qualification in Social Work, Human Services, Behavioural Science, Social Science, Psychology, Criminology, Allied Health, AOD, Occupational Therapy, Sociology, Public Health, or a relevant field.
• Ability to apply high-level practice judgement when working with clients to set and achieve goals.
• Experience working with culturally and socially diverse clients, applying culturally responsive frameworks.
• Ability to record accurate and consistent case notes.
• Ability to work independently and collaboratively within a multidisciplinary team, contributing to innovative service development.
• Willingness to travel across the ATSICHS Brisbane region and work at different sites.
• Willingness to provide social work support outside of clinic environments.
• Hold or be eligible to obtain a Working with Children's Card (Blue Card), NDIS Working Screening Card (Yellow Card), and a National Police Clearance.
• Driver's Licence.

Preferred :

• Experience in a multidisciplinary team environment.
• Experience working in an Aboriginal Medical Service.
