What You'll Do:

  • Perform comprehensive physical therapy evaluations and develop individualized care plans
  • Deliver engaging, evidence-based therapy sessions that promote mobility and strength
  • Collaborate with care teams to support whole-person recovery
  • Educate residents, families, and fellow team members on techniques and goals
  • Track progress, update treatment plans, and document results
  • Create a safe, positive environment focused on dignity and achievement

What You Bring to the Team:

  • Degree from an accredited Physical Therapy program
  • Active PT license in the State of Indiana
  • Strong communication and teamwork skills
  • A patient-first mindset with a drive for results
  • Experience in long-term care or rehab settings is a plus—but not required
