Laurence (BSL), we are seeking a passionate Relief Care Partner to join our dedicated team Aged Care team.
Location : Frankson Job Type : Permanent Full-Time working 38hours per week Salary : $87,886.87to $$94,565.36per annum, based on skills and experience plus 12% superannuation.
(pro-rata for part-time employment) What we offer our employees Salary packaging options to improve your take home pay – up to $18,500 can be packaged every year (details via ) Career development opportunities Leave loading of 17.5% Flexible working arrangements Employee Assistance Program Discounted health insurance and free flu shots each year About the role The Relief Care Partner provides essential coverage for Care Partners during periods of planned or unplanned absence, ensuring continuity and consistency in the delivery of person-centred care to participants in the Community Care program Key Responsibilities Be the primary point of contact for participants and their registered support and / or their family members, identifying participant needs, goals, personal preferences, and existing supports, and provide timely information on available services Communicate with relevant stakeholders involved in the delivery of services regarding the participant's needs and wellbeing to action the care plan and associated recommendations in a timely manner.
Work within a team-based model of care, ensuring clinical decisions are escalated appropriately and timely, and participate in shared planning and problem solving to improve the participants' health outcomes.
Support participants to make informed decisions, including respecting their right to take risks, engaging with registered supporters and carers as appropriate, and as per the supported decision-making framework.
Support participants, their registered supporters, and family members to navigate age-related systems and programs, linking them to additional support in line with their care plan needs and goals About Brotherhood of St Laurence We are passionate about the work we do.

What we are looking for We are seeking an enthusiastic and motivated individual who has a passion for supporting the mission of BSL and delivering excellence in your role.
To be successful in this role, we are looking for someone who has the following key skills and experiences : Tertiary qualification in Health or a related field (including Individual Support – Ageing; Health Services Assistance; Aged Care; Disability; Community Services Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience Formal training in trauma-informed care, cultural safety, or restorative practice Strong background in aged care - home care, care management, or a healthcare setting.
Sound understanding of compliance in Aged Care and NDIS, including quality standards and Work, Health and Safety (WHS) regulations.
