About Us
Right at Home provides consumer‑directed, in‑home aged, disability, and nursing care services.
Our new Newcastle Toronto office is in its establishment phase, and we're seeking a committed Care Manager (with current AHPRA registration) to champion our culture and values.
Role Overview
As Care Manager you will:

Lead clinical oversight and in‑home assessments, creating and reviewing personalised care plans
Recruit, train and mentor care staff, ensuring high standards and regulatory compliance
Manage rosters, KPIs and quality initiatives to drive continuous improvement
Build relationships with clients, families, healthcare partners, and local community organizations to support service growth and promote our services

Key Responsibilities

Clinical Leadership:

Ensure care delivery aligns with Right at Home policies and AHPRA standards

Care Planning:

Conduct home visits, assess needs and develop care plans

Team Development:

Recruit, train and manage staff on clinical competencies, and foster a supportive culture

Quality & Compliance:

Monitor performance, investigate incidents and implement improvements

Stakeholder Engagement:

Liaise with referral sources and coordinate client communications

Qualifications

Qualifications & Experience

Health Science degree (essential); postgraduate or specialist certificates (desirable)
Demonstrated knowledge of Aged Care Quality Standards and continuous quality improvement

Preferred Skills

Key Selection Criteria

Current AHPRA registration (RN, Physio or OT)
Driver's license, comprehensively insured vehicle, and police clearance
Minimum 3 years' post‑registration experience in aged or community care
Proven ability in assessment, care planning and team leadership
Excellent interpersonal, communication and problem‑solving skills
