About the role Reporting to the Directors at My Plan, My Way, this full-time NDIS Support Coordinator role in Toowoomba, QLD offers an exciting opportunity to make a meaningful difference in the lives of people living with disability.
As an NDIS Support Coordinator, you will be responsible for guiding participants through the NDIS process, developing tailored support plans, and connecting them with essential services and community resources.
What you'll be doing

Develop and implement person-centred support plans in collaboration with NDIS participants
Coordinate access to a range of support services and community resources to meet individual needs
Advocate on behalf of participants to ensure they receive the supports and services they're entitled to
Provide ongoing monitoring and review of support plans, making adjustments as required
Maintain accurate and up-to-date records and reporting in line with NDIS requirements
Build strong relationships with participants, their families and other stakeholders
Contribute to the continuous improvement of service delivery at My Plan, My Way

What we're looking for

Tertiary qualification in a relevant field such as Social Work, Disability Services or Community Services
Demonstrated experience as an NDIS Support Coordinator or similar role in the disability sector
Excellent communication and interpersonal skills, with the ability to empathise and build trust
Strong problem-solving and negotiation skills to effectively advocate for participants
Thorough understanding of the NDIS and relevant legislation, policies and procedures
Ability to work both independently and collaboratively within a team environment
Willingness to travel within the local area to meet with participants
