We're on the hunt for our next Aged Care Program Officer to be the connector who makes things happenPlease note this role is a part-time (24 hours per week), and is contracted until 30 June 2027.

What You'll Do You'll support strong partnerships and practical improvements between aged care homes, pharmacists, and general practice.
Your day-to-day might look like: Build Connections: Pair up aged care homes with local pharmacists and nurture trusted partnerships.
Keep Projects Moving: Facilitate smooth communication and collaboration across the system.
Grow Local Networks: Organise forums, training, and communities of practice to spark shared learning.
Share What Works: Distribute practical resources, updates, and tools across the region.
Champion Digital Health: Promote use of digital tools and provide education to support uptake.
Bring Programs to Life: Help implement SEMPHN initiatives on the ground - efficiently and impactfully.
Support Improvement: Contribute to reporting and evaluation so we can learn, adapt, and improve.

You're a great communicator, open to feedback, and love working with different perspectives.
Are equally comfortable flying solo or teaming up to get things done.
