Overview


We have a great opportunity for multiple Direct Care Workers to join our dedicated Community Health team in providing a direct client service to persons who are aged, frail or disabled who wish to remain living in their own home with some assistance.


The Direct Care Worker (Community) is accountable to the Community Care Coordinator and works in close collaboration with Community Health professionals and is part of a multi-disciplinary team.


About You

We are looking for applicants with:

  • proven ability to work well within a team environment;
- effective interpersonal and communication skills;
- ability to relate to people from different cultures, backgrounds and circumstances;

  • Proven ability to meet deadlines and timeframes;
  • Ability to provide assistance and cooperation to other staff;
  • Ability to use discretion and maintain strict confidentiality; and
  • Ability to perform work of a general nature under supervision.
Limited experience working with aged, frail or disabled persons in a community/hospital setting needed.
