We are currently seeking a committed
Care Planning Coordinator to join our Clinical Leadership team at our
Moran Home at Kellyville.


Reporting to the Care Manager you will be responsible for the overall clinical assessment program, and supporting the RNs as required to ensure resident assessments are completed and inform the development of care plans tailored to resident's individual assessed care needs.

As a member of the Clinical Leadership team, participates in clinical governance activities, including planning and implementing clinical priorities.

About the Role

  • Working closely with the Care Manager (CM) you will establish a Care Plan review program ensuring care plans and assessments remain current and relevant, reviews are initiated when required and residents assessed care needs are well documented.
  • Participating in the development of training and education plans to ensure employees are provided with the required support and education to enable them to competently complete assessments, care plans and documentation based on accurate and comprehensive assessment, consultations, observation, and collaboration.
  • Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship and reporting and consent obligations.
  • Assessment and care planning for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals and Admissions Coordinator to ensure a personalised care plan is developed.
  • Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices and take action as required if gaps or inconsistencies are identified
  • Review of care incidents and care change patterns for residents and collaborate with Clinical Care Team to determine if further assessment and care planning modifications are needed, initiate care plan reviews and reassessments
- maintain accurate client records, case notes and other data as directed and required.

About You

  • Registered Nurse with at least 2 years clinical experience; aged care management experience and familiarity with the Quality Standards for Aged Care
  • A high level of knowledge and understanding of the Care Planning, Assessment and Documentation in Residential Aged Care
  • Excellent and demonstrated verbal, written and interpersonal communication skills.
  • Knowledge and ability to use computers and technology efficiently.
  • Current Police Check and appropriate Working Rights
  • Up to date vaccinations (Influenza, Covid)
  • Experience in a similar position in Aged Care desirable
  • Management qualifications highly regarded
