Your Opportunity
Based in Karratha WA, as a Housing Case Manager you will provide support to our THRIVE program. THRIVE supports tenants of public housing and Aboriginal housing who experience difficulties sustaining their tenancy, while addressing the issues associated with their homelessness and current circumstances.
You will work with individuals and families supporting them achieve their self-identified goals with a holistic, person-centred, culturally aware, and solution focused approach.
Your role will be dynamic and proactive, as you actively identify and address barriers to engagement, ensuring clients are equipped to transition into sustainable housing and achieve independence. From providing supportive services to those experiencing homelessness to assisting those at risk of homelessness, you'll play a pivotal role in creating pathways to stability and empowerment.
By forging connections with external services and resources, you'll help address complex barriers related to housing tenure, homelessness, education, and employment opportunities.
We have two positions available in this incredible service. 12months fixed term parental leave cover and a permanent position.
Your Key Responsibilities
- Respond to referrals from the DoC-H as well as individuals, families, local Mission Australia programs or other external agencies in a timely manner.
- Work with individuals and families to develop a holistic case plan that is sensitive and responsive to their cultural strengths and identified needs and goals to increase tenancy outcomes, support networks, independence, and wellbeing.
- Conduct regular home visits to provide therapeutic interventions, such as informal counselling, based on the specific needs and issues identified by individuals and families.
- Facilitate and develop relationships with organisations and services that work with and support people living in public housing.
- Ensure that all required internal and external client paperwork, including individualised Client Management files, is completed and copies kept in accordance with MA policies and procedures on file.
Requirements for Success
With a significant background in the housing, homeless and/or human services sectors, you will be confident in your case management and assessment processes. Other requirements include:
- Minimum qualification of Certificate IV in Community and/or Human services or equivalent skills and experience.
- Demonstrated experience working in a human services organisation showcasing relevant case management including assessment, case work, accurate and timely recording of case notes, reviews, advocacy, and referrals to services if required.
- Ability to work empathically with individuals and families using a strengths-based approach, while using initiative and working independently.
- Excellent communication and interpersonal skills including, with strong computer literacy in Microsoft Office suite, and other systems required to support the work.
- Knowledge of complex and challenging issues that place people at risk of homelessness or tenancy stress.
- Sound knowledge and understating of the Aboriginal and Torres Strait Islander cultures.
- Must have Full driver licence with no restrictions.
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
