What you'll be doing

  • The Intellectual Disability Care Coordinator for Central Coast Local Health District is responsible for providing expert advice to support health staff to deliver patient / clientfocused care to people with intellectual disabilities and their families / carers.
  • The Intellectual Disability Care Coordinator position will lead the development of appropriate clinical practice and care for patients/clients with intellectual disability and their carers and families in any hospital or healthcare setting in the local health district e.g. community health or outpatient setting.
  • The position will work collaboratively with the Hunter New England Specialised Intellectual Disability Health Team to build capacity in local health services, and assist with developing and delivering outreach services in Central Coast Local Health District.
  • Outreach services will include multidisciplinary assessment and care planning for people with intellectual disability and complex health needs, and support activities to build the skills, confidence and experience of mainstream clinicians when working with people with an intellectual disability.
