About the Position

The Team Leader plays a crucial role in supporting the Care and Support team members to deliver top-notch care to our residents consistently. Ensuring that each resident's unique identity, culture, diversity, beliefs, and life experiences are acknowledged and respected at all times. Working closely with the Clinical Care Coordinator and Registered Nurse to supervise and organize resident care throughout the facility, while prioritizing individual needs, preferences, and aspirations. The Team Leader collaborates within a cohesive team following an established framework to empower residents to live their best lives.

Main responsibilities include:

  • Direct involvement in resident care to meet individual needs and care plan requirements
  • Providing emotional and social support to residents, focusing on holistic and person-centered care
  • Assisting in coordinating residents' care needs such as medication, nutrition, and pressure care management
  • Identifying and reporting any resident concerns to the Registered Nurse or Clinical Care Coordinator
Desired Skills and Qualifications
  • Holder of Certificate IV in Ageing Support, including HLTHPS007 Administer & Monitor Medicines OR Diploma of Enrolled Nursing with APRHA Registration (endorsed) OR Diploma of Enrolled Nursing (non-endorsed) with HLTHPS007 Administer & Monitor Medicines
  • Demonstrates a high level of commitment, honesty, integrity, and trustworthiness
  • Strong communication skills, both verbal and written, with active listening abilities
  • Proficiency in utilizing information technology, adaptable to learning new skills as needed
  • Current National police check and NDIS Worker Screening check
  • Familiarity with Workplace Health and Safety (WH&S) and Infection Control practices
