Your Role

As an Assistant Co-ordinator - Community Care you'll be looking after the most important people in our organisation - our clients.

Your tasks will include:

Assess, plan, co-ordinate and review care deliveryEnable clients to maintain as much independence as possible, and encourage them to use their abilities and initiative to develop existing and new skillsReport and document changes in behaviour patterns, unusual actions or reactions of clients or employeesProvide direct client care when requiredAssist with scheduling of employees to suit the needs of the clientLiaise and network with other service providers and the community as directed

Role Requirements

Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)Current Police Screening CertificateCurrent First Aid and CPR CertificateCurrent NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)Current Queensland driver's licenceTertiary Qualifications (as appropriate) and/or relevant experience in a health-related field (e.g. Health and Human services)Experience in case management and client care co-ordinationDemonstrated sound verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of peopleFlexibility to work across a range of days (Monday to Sunday)
