Monash Health is excited to announce a new position as part of the expansion to the Hospital Outreach Post-suicidal engagement (HOPE).


About HOPE:


The HOPE program provides psychosocial support for people who are identified as having presented with suicide or self-harm attempt, or suicidal or intentional self-harm ideation.

The initiative aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and targeted psychosocial support.

The aim of the initiative is to prevent avoidable suicide attempts and reduce the number of completed suicides by providing targeted psychosocial support services to a cohort of people in an assertive manner in the community for up to 3 months from the time of discharge.

About The Role


Within a relationship of mutuality and information sharing, Family/Carer Peer Workers use their own lived experience of caring for someone with mental health issues to provide emotional support, focus on family/carer wellbeing, and promote choice, self-determination and greater opportunities for family members and carers to be heard.

Family/Carer Peer Workers work as part of a multidisciplinary team and establish strong, effective working relationships with clinical, allied health, and Consumer Peer Work staff.


Family/Carer Peer Workers act as recovery champions within the mental health program and may be involved in other activities including training and education, improving experience of care and service evaluation.


Families/Carers follow up both by phone and face-to-face contacts within the community, or the family/carer's own environment in an assertive outreach manner.

What You Need

  • Demonstrated experience
- lived experience knowledge and skills

  • Knowledge of peer support principles
  • Experience working in lived experinece role

  • Lived and/or living experience of caring for a person who has experienced serious mental health issues and has used mental health services
  • Recognition and understanding of own personal experience and ability to use this appropriately to support others. Ability to use own experience to inspire hope and belief in recovery
  • Experience working in a Family/Carer lived experience role
  • Experience of providing supervision, and
