We are currently seeking a dedicated and compassionate Disability Services Officer to join our team, providing high-quality support and services to individuals with disabilities. This role is an incredible opportunity for a professional who is passionate about making a difference in the lives of others and is committed to promoting independence, empowerment, and inclusion.

**Key Responsibilities**:

- Develop, implement, and monitor individualized plans to meet the needs and goals of service users.
- Provide direct support to individuals with disabilities, including personal care, social support, and assistance with daily living activities.
- Facilitate and promote the participation of service users in community activities and events.
- Work collaboratively with service users, families, carers, and other professionals to ensure a coordinated and integrated approach to service delivery.
- Maintain accurate records and documentation in accordance with organizational and legislative requirements.
- Participate in continuous improvement activities and training to enhance service delivery.

**Essential Criteria**:

- Diploma qualification in Disability Services, Community Services, Mental Health, or a related field.
- Proven experience in providing support to individuals with disabilities, including developing and implementing individualized plans.
- Excellent communication and interpersonal skills, with the ability to build rapport with service users, families, and colleagues.
- Strong organizational and time management skills, with the ability to prioritize tasks and work independently.
- A commitment to the principles of social inclusion and the empowerment of individuals with disabilities.
- Current First Aid Certificate and Working with Children Check.
- NDIS Workers Clearance
- Valid driver's license and reliable transport.

**Benefits**:
