Recruitment is currently seeking a Coordinator / Case Manager for a Not for Profit delivering a specialist community-based aged care and disability program in Inner Western Sydney.


his fantastic opportunity will support older adults and people with a disability to assist participate in community life by meeting their need for social contact and accompaniment.

Activities include health and wellbeing programs, outings, and intergenerational programs. This full-time position is on a temporary contract with the possibility to be made permanent. The role works with a supportive and collaborative team.

Previous leadership experience in aged care or the disability sector and knowledge of social support is mandatory to succeed in this rewarding role.

This program is a community where seniors connect and share their knowledge to lead active, social and independent lives.

Duties:

  • Deliver and manage social support communitybased programs for older adults in the community
  • Build key working relationships with both internal and external stakeholders
  • Support the management team to ensure the program runs inline with the Commonwealth Home Support guidelines and aged care quality standards
  • Participate in leadership duties
  • Maintain clear and precise client records and data management skills
  • Work collaboratively in a team and independently

Skills and experience required:

  • Minimum of two years' experience in a team lead style role, ideally in a communitybased program & previous experience in a support style role
  • Tertiary qualification in Social Work, Psychology or similar
  • Current NSW driver's license is essential
  • NSW Working With Children Check
  • Valid First Aid Certificate and CPR
  • Fantastic interpersonal skills and emotional intelligence
