JOB PURPOSE


To support HCS NDIS participants by providing quality services using a person-centered approach in their own homes in their day-to-day living, ensuring that they have maximum and optimum opportunity for developing valued and valuing lifestyles.

Professional engagement with NDIS participants is concerned with generating quality personal relationships, which are supportive and life enhancing.

Provide opportunities for an enabling environment that facilitates choice & control, self-advocacy and participation in matters relating to the home as well as facilitating access to community services and facilities.

To support NDIS participants with all aspects of everyday living.


Under the direction and instruction of the Team Leader, this role will be responsible for providing a level of support to clients in a range of disability programs.

Through your work, you will enable participants to achieve their personal goals and participate as valued members of the community, while you gain hands-on experience towards a formal qualification in Disability or related field that you may consider during employment or on your own terms.


KEY STAKEHOLDER RELATIONSHIPS

INTERNAL STAKEHOLDERS CEO, board members, all HCS Team Leaders, administration officers, and staff involved in

HCS' group homes, Respite staff & or Day

Program.

EXTERNAL STAKEHOLDERS External contractors of therapists, government authorities, court and police representatives, families, service providers and suppliers.

2. SERVICE DELIVERY KEY RESULT AREA (KRA):_
SUPPORT SERVICE_

Support clients to participate in age appropriate activities and tasks

including in home support and community access. Perform duties in

accordance with organisational requirements.

Performance measures:

  • High level of support and care provided to the client in
accordance with their ISPs and NDIS goals.

  • Action and attend all scheduled client appointments occurring within your allocated shift.
  • Implement behaviour management and deescalation strategies
in accordance with client BIS to continuously manage

behaviours of concerns.

  • Medication accurately administered during shift.
  • Full compliance with HCS Code of Conduct.
  • Full compliance with WHS policy.
  • Respect clients' rights and ensure they are not breached.
  • Maintain professional boundaries on shift.
  • Respect the rules and expectations of the Group Home.
Provide direct care assistance to each client in accordance with theirservice plan and as directed by Team Leader.

Performance measure:

  • Provide and implement assistance for the client/s with the following: personal care needs each morning and evening,

organise time/day and assist with structured weekly routine,

prepare meals, travel and transport to appointments, community

access, administer medication and health care plan, and

implement ISP and support plans.

Monitor safety and well-being of participant.

Performance measure:

  • Report any issues that may compromise the safety and well
- being of the participant/s directly to their TL immediately.
