About the role

The Senior Practice Coach role will be responsible for leading the development and execution of targeted discreet and BAU initiatives that drive practice improvements for front-line staff across a wide variety of social service teams in the Human Service branch. The role combines a level of systems activities and front-line higher level leadership responsibilities. The role will report to the General Manager of Human Services and lead higher-level capability building transformation.

Duties

  • Meet individual staff case performance targets as per weekly, monthly and quarterly dashboard system. Meet project management targets for all practice initiatives led and owned.
  • Lead the development of a systematised and automated approach to staff practice, in line with the program logics and models of care.
  • Lead the development, implementation and ongoing delivery of the Practice Skills Acuity Program, focusing on improving the practice interventions and professional behaviours of staff.
  • Case Manage staff to provide intensive one-to-one practice improvements to support, ensuring selection of staff are supported based on lowest client outcome performance across branch.
  • Lead the development and ongoing execution of rigorous risk management practices across Human Service teams, particularly in Youth Justice and Child Safety Out of Home Care and Statutory Interventions.
  • Lead the development and implementation of care coordinated systems and approaches that improve client pathways, particularly in providing wrap-around care across multiple needs of a family. Personally lead the direct case management of extreme-risk cases where risk is identified as extremely hazardous to the brand reputation to ATSICHS.

  • Starting Salary of $95,000 per annum (depending on qualification and experience) + 11% Super + Not for Profit Salary Packaging (Increases your take home pay).

COVID - 19 Requirements

Required

  • Current Bachelor's Degree, in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention or Criminology with 6 years' relevant experience in child protection, youth justice or mental health field.

OR

  • Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 8 years' relevant experience in child protection, youth justice or mental health field.
  • At least 5 years experience in social services.
  • Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
  • Demonstrated ability and theoretical knowledge of client agency, self-efficacy and facilitating change through client empowerment, while actively discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
  • Demonstrated ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs.
  • Demonstrated ability in coordinating multiple services in providing access to holistic care for clients, particularly integrating internal agency supports across multiple teams.
  • Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.
  • Experience with case management for high risk and/or crisis clients, with high-level planning and organisational skills.
  • Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment and housing.

Additional Requirement

  • Requirement to travel across the Logan and/or Brisbane region, as per program location.
  • Current drivers' licence.
  • Requirement to work outside core 9am-5pm business hours in meeting individual client needs and provide responsive care.
  • Working with Children's Card (Blue Card) or be eligible to apply.
