About the position:

  • Location: Ashfield Terrace Care Community
  • Lead the Meaningful Life program at the Care Community
  • Create opportunities for meaningful engagement, purposeful participation, social events, and activities for all residents
  • Drive Opal HealthCare initiatives including Meaningful Mates and Helping Hands
  • Design programs to enhance residents' physical, emotional, and spiritual well-being along with their overall quality of life
  • Develop resident care plans and perform key lifestyle assessments
  • Collaborate with clinical teams and allied health professionals to provide a personalized approach for each resident to improve their mobility and independence
  • Guide care team members to facilitate meaningful interactions among compatible residents
  • Coordinate with residents to establish activities and engagements that align with their cultural, spiritual, social, and physical needs and preferences
  • Interact with residents' families to promote visitation and socialization
  • Demonstrate a Person-Centered Care approach
  • Qualifications in Leisure and Health/Community Services are beneficial
