Overview
Joining the Out of Hospital Team within the Community Health Service, you will use your clinical knowledge and professional competence to plan, implement and evaluate a comprehensive and integrated range of social work services appropriate to the needs of the Barossa/Gawler community.
As a member of the multidisciplinary team, including health professionals and service providers from other sectors, you will utilise a combination of preventative, early intervention, treatment/therapy and evaluation approaches.
You will support clients, carers and families across the patient journey, providing effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow-up.
You will provide interventions for clients across multiple age groups and health settings, including Community Health, NDIS, CHSP, Acute Care and care pod settings.
Working under reduced clinical direction, you may contribute to the clinical supervision of less experienced allied health professionals, allied health assistants and students.
