As a Care Facilitator serving clients in the Inner West/South East Sydney and neighboring areas, you'll aid clients and their families in accessing services to help them stay at home. You'll empower our clients through personalized care planning and goal-setting focused on a well-living approach.

A typical day will involve:

  • Visiting clients at home in the community referred for assistance
  • Contributing to the creation, execution, and assessment of services and programs tailored to clients' specific needs
  • Building relationships with families and caregivers to address their requirements
  • Collaborating with clients to establish realistic and achievable goals
  • Applying best practices and evidence-based methods for care in close cooperation with a multidisciplinary team
  • Recording information and using client record management systems
  • The role will aid clients in the LGA's of Inner West/South East Sydney and nearby regions

What we expect from you:

You should feel at ease working autonomously in the community, possess excellent verbal communication skills, and exhibit an empathetic, patient, and caring demeanor.

This position requires:

  • Bachelor's Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management) or equivalent knowledge and experience
  • Experience in aged care or relevant transferable skills will be highly valued
  • Demonstrated proficiency in operating under financial constraints and achieving service delivery goals within government program parameters
  • A valid NSW driver's license
  • Intermediate computer skills
  • Strong time management and organizational abilities
  • Capability to work independently and collaboratively within a team
