About the role..

Are you ready to take the helm in guiding patients to the right care at Royal Rehab Private Petersham (RRPP). As our Rehabilitation Coordinator/Hospital Bed Manager, you'll lead the charge in reviewing, coordinating inpatient referrals and ensuring seamless bed allocations for our patients. Collaborating closely with members of the executive and multidisciplinary team, you'll play a pivotal role in orchestrating the perfect balance between patient needs and hospital resources. Moreover, you'll spearhead efforts to cultivate and nurture relationships with our valued referrers, paving the way for growth and success at RRPP.

Key responsibilities include:

  • Assess patients that have been referred to Royal Rehab Private Petersham for appropriateness of a rehabilitation admission.
  • Coordinate rehabilitation referrals to hospital bed availability.
  • Advise the other members of the executive and multidisciplinary team of suitable bed allocations, which aim to meet both the patient and hospital needs.
  • Provides monitoring and surveillance, follow-up phone calls and assistance to navigate access or re-entry into any service at RRPP, along the continuum of care.
  • Actively supports continuous improvement through participation in quality activities.
  • Collects and maintains statistical records and information as it relates to referrals, admission number and type.
  • Keeps abreast of current research, promotes innovation, and seeks best practice rehabilitation opportunities for implementation at RRPP and assumes a leadership role in the implementation of those opportunities.
  • Identifies new referral sources and markets RRPP services.
  • Develops links with referring hospitals to assist with smooth patient flow transition.
  • Develops an awareness and links with community services and support groups to assist with facilitating access to Short Term Accommodation services (STA).
  • Promote and manage STA services.
  • Liaise with 3rd party insurance and completes approval requests for patient admission and ongoing care.
  • Develops marketing activities focused on specific holidays of the year such as Christmas and Easter. Liaise with the marketing team on promotional brochures, website content and google advertising.

About you..

We are seeking a self-motivated and friendly individual with:

Essential criteria:

  • Registered Nurse or Allied Health professional (AHPRA).
  • Minimum 5 years full time equivalent post registration experience. With at least 3 years full time equivalent in the field of Rehabilitation/Orthopaedics, Cancer and Cardiac Reconditioning in a hospital setting.
  • Demonstrated ability to communicate effectively with all staff, patients/clients and visitors.
  • Demonstrated ability to participate within a multidisciplinary team environment.
  • Highly developed skills in marketing private hospital health services to internal and external customers.
  • Strong clinical assessment skills.
  • Demonstrated ability to work autonomously, with a can-do-attitude.
  • A commitment to professional development.
  • Computer literacy.
  • Excellent customer service skills.
  • Class C Drivers licence.
  • FIM trained or willingness to complete training.
