Royal Rehab Private Petersham is part of the Royal Rehab Group, a leading provider of disability and rehabilitation services.

A 34-bed private rehabilitation hospital centrally located in Petersham NSW, Royal Rehab Private Petersham is close to public transport and with access to off street parking.

We provide the highest level of patient care through dedicated staff experienced in all major rehabilitation specialties. Multidisciplinary care is delivered to patients according to individualised patient goals and needs.

About the role.

Exciting opportunity for a talented Neurological Care Coordinator to join our dedicated team at Petersham. This position will facilitate the continuity of care for patients with a neurological diagnosis and their caregivers.

Key responsibilities include:

Assist the patient to navigate through their rehabilitation journey across all services i.e. from inpatient rehabilitation, day program, discharge to the community and re-entry at any point as required.
Be the link between the patient, their families, health care professionals, and community partners.
Be a proactive member of the multidisciplinary team who contributes to the person's rehabilitation goals.
Collect data referencing the Stroke Guidelines and ensure compliance with evidence-based best practice.
Support the development of knowledge and skills in significant others and carers through teaching and coaching.
Provide education and self-management interventions to patients and their families/carers to support their ongoing rehabilitation.
Develop strong relationships with community Neurological services to promote services and develop awareness raising events and activities.
Assessment of neurological patient referrals.
Monitors and actively participates in the pursuit of quality health care practices.
Work closely with the Executive team on Marketing.
Monthly reporting for the Director of Clinical Services.

About you.

We are seeking a self-motivated and friendly individual with:

Essential criteria:

Registered Nurse or Allied Health professional (Occupational Therapy or Physiotherapy) with current APHRA registration.

Minimum 5 years full time equivalent post registration experience with at least 3 years full time equivalent in the field of Rehabilitation/ Neurology.

Demonstrated ability to communicate effectively with all staff, patients/clients, visitors.
Demonstrated ability to participate within a multidisciplinary environment.
A commitment to professional development.
Computer literacy.
Excellent customer service skills.
