The Shoalhaven Transitional Aged Care Program is seeking motivated Level 3 Social Workers. The role supports older clients to return home after hospital, to build capability, re-condition, increase independence and confidence and avoid early entry into residential care. Social workers are an integral part of the MDT providing psychosocial assessment and therapy, counselling, case management and advocacy.

What you'll be doing


Provide specialist Social Work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

This position provides the above Social Work service to the Shoalhaven Transitional Aged Care Service (STACS). STACS provides a time-limited, goal orientated service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision making process around long term care

The social worker will also maintain social work and case management responsibility for a number of cases until discharge.

Duties include early psycho-social assessments, care planning, education, advocacy for the needs of clients, families and/ or carers, liaise with community agencies, the client's General Practitioner, counselling and carer support.


The position functions as part of an inter-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care, but are unable to return home without additional care and support.

There is a strong working relationship with the Illawarra Shoalhaven Aged Care Assessment Team (ACAT) and with a Site Social Work Unit Head for professional supervision.


All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (3 doses for Category A roles, 2 doses for Category B roles) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

  • Extensive post graduate experience in the provision of Social Work services in a health related setting
  • Demonstrated skills in case management with high level assessment and intervention skills and the ability to manage own caseload through coordinated assessment and care management of older people with complex needs.
  • Extensive knowledge of issues related to working with older people and knowledge of Commonwealth Government guidelines on aged care residential assessments, the Aged Care Act and Transitional Aged Care Programs.
