We are seeking:

Complex Care Manager
Fixed term (12 months) parental leave position from 13/2/ /2/2024:

Role will include working with clients in the community and some cross campus work.

About the Complex Care Management Team:


The 4 specialist services of HIP Complex Care Management aim to assist people with chronic disease, aged and/or complex needs who frequently use hospitals or are risk of hospitalisation.

Our intention is to assist these vulnerable people to remain living at home and to prevent unnecessary representation to the Emergency Department or admission to Hospital.

The service operates from Monday to Friday.

To be successful in this role you will demonstrate:

  • Allied Health Professional or Registered Nurse with relevant registration to your professional body.
  • A passion for working in a multidisciplinary team
  • Support for client centred care planning and provision of care
  • Great verbal and written communication skills and report writing
  • Experience in coordination of service provision.
  • Advanced casework skills, including complex assessment and care planning skills, client education and service linkage.
  • Sensitivity to the needs of patients from culturally and linguistically diverse backgrounds.
  • Welldeveloped workload and time management skills.
  • Client Centred approach to service delivery.
  • Commitment to professional development and evidencebased practice.
  • Ability to problem solve in a variety of complex situations.
  • A current Victorian Driver's Licence and willingness to drive to home visits.
