At MSWA, we are dedicated to offering tailored care and support for individuals with neurological conditions such as MS, Parkinson's, Huntington's, Motor Neurone, Acquired Brain Injury, and Stroke. Our comprehensive services include Allied Health/Clinical care, In-home care, and High-Support Accommodation.

We are seeking an experienced Coordinator to join our team based in our Bunbury office. You will manage a caseload of Aged Care or NDIS funded clients, connecting them to internal and external services, guiding them through their Aged Care or NDIS journey, and assisting them in achieving their defined and funded goals.

Your daily responsibilities will include:

  • Developing and implementing service agreements.
  • Addressing feedback about services and assessing service referrals to ensure funding eligibility.
  • Monitoring client budgets and costings.
  • Conducting in-home client visits to better understand your clients.
  • Ensuring compliance with key Aged Care and NDIS standards.


In this role, you will make a significant impact, leading to positive and successful client outcomes.
Please note that travel to client's home is part of the role, pool cars are available for use.

Essential Requirements

  • Experience or qualifications in Disability Services or Aged Care Services, ideally within a similar role.
  • Excellent interpersonal and communication skills.
  • A genuine desire to make a difference.
  • NDIS Worker Screening Check (or willingness to obtain).
  • Current Australian "C" Class Drivers Licence with no restrictions.
  • Exceptional time management and organisational skills to prioritise your workload and meet conflicting deadlines in a busy environment.
