About the role


An exciting opportunity to cover a maternity leave position as Team Leader, Ageing & Social Support and to join our thriving community health organisation and work within a truly supportive and valued based culture and engaged workforce.


As the Team Leader, Ageing & Social Support you will lead the operational and strategic development of the Ageing and Social Support program and lead the transition to Support at Home in the carer respite, allied health and social support programs in response to changing directions in community based aged services, community preferences and Access Health & Community's Strategic objectives.


The role will ensure compliance with contract requirements and quality standards in the delivery of the contract requirements and develop internal and external partnerships that extend the reach and quality of the program and play a leadership role in designing, delivering and leading reforms.


What you will be doing

  • Ensure the program is appropriately staffed with qualified and experienced staff from diverse backgrounds to meet community needs and funded targets
  • Ensure direct care staff are deployed and supported to flexibly meet the needs of our clients
  • Oversee daytoday supervision and onthejob training and support for support staff
  • Lead a collaborative team culture that consults staff values and develops their expertise
  • Manage and oversee the delivery of diverse and interesting program of activities within funding guidelines to meet participants' needs in consultation with the team leaders
  • Keep abreast of community needs and changes in Government policy directions and ensure the program is responsive to these
  • Develop internal and external partnerships to establish diverse referral pathways, gauge and respond to community needs, and add value to the program
  • Proactively monitor and report on expenditure relating to projects within budgets

What you will bring

  • Key Selection Criteria_
  • Relevan
    t tertiary qualification or substantial work experience in community aged care
  • Minimum of five years' experience in a Team Leader role within aged or adult disability care
  • Experience in program design and innovative delivery in the community aged care sector
  • Successful engagement and leadership of teams to embrace changes required to successfully navigate the evolving health care landscape
