Key Responsibilities
Provide support to participants with high Physical and Challenging Behaviours
Provide personal care to persons with disability
Maintain all written correspondence and reports up to date within required standards

Execute as instructed designated tasks and activities aimed at meeting the clients' personal needs/community support/empowerment, team objectives and/or program goals.

Fulfil designated duties/tasks as instructed by the employer when requested/required.
This includes working across all client groups when and if required.
Participate in regular team meetings.
Carry out duties/activities in a cooperative, respectful, and flexible manner.
Adhere to the service/facility communication, reporting and participant/program progress practices and procedures.

Respond to crisis in accordance with specified guidelines (Policies and Procedures, Code of Conduct, Incident Reporting) and seek assistance to situations outside of specified guidelines.

Participate in developing and monitoring individual program plans and be accountable for their outcomes.

Establish good working relationships and work closely and collaboratively with team members to carry out the duties and responsibilities of the role.

Providing support to colleagues when required.
Participate in on-the-job training/site induction when required and/or requested by your manager.
Participate in training and development opportunities to enhance skills and performance.
Establish good working relationships and networks with other community organisations to keep abreast of community activities and initiatives.
Work under general guidance from the participant and their family seeking support, within clearly defined guidelines.

Qualification:
Relevant Diploma or higher qualification in health and wellbeing; and
At least two (2) years, full-time work experience.

Proactive and passionate about supporting people with disability
Flexible
Team player
Effective communicator
Ability to work with minimal supervision

You musthave the NDIS Worker Orientation certificate completed and please log in if you haven't obtained the certificate already.
