Position:


An exciting opportunity exists for a NDIS Support Coordinator to join our team at Simply Helping Gippsland South and West (SHGSW) on a full-time basis.


The primary purpose of the role will be to ensure that NDIS support coordinated participants are proactively assisted in optimising the use of their NDIS plans and aligning their budgets and goals.

You will also assist NDIS participants to engage with support services, Allied Health practitioners and social and community activities, building on their current informal supports, and guiding them through the NDIS process.


Key Responsibilities:

  • Manage an assigned case load
  • Engage and support NDIS clients to meet their plan goals within the NDIS framework
  • Conduct and maintain uptodate goal tracking and risk assessment
  • Provide appropriate information and advice to NDIS participants, sighting official NDIS publications where necessary
  • Complete service agreements and care delivery plans
  • Liaise with internal and external service providers to maintain continued support for participants
  • Connect participants to appropriate external allied health services
  • Maintain communication with external/internal services to ensure continuity of care for participants
  • Monitor budgets and work with participants where the need for a change of circumstance is required
  • Support participants in providing NDISrequested evidence for reviews
  • Ensure NDIS participant records are strictly maintained and kept uptodate
  • Remain uptodate with relevant legislation and industry regulations

A NDIS participant would like to go on a holiday with a support worker. The location is not far from where they live, and it would only be for 2 nights. They are supported for social and community most days as they are independent and mobile.
