As a
Disability Care Coordinator, you will be responsible for ensuring that our patients receive high-quality care, with a focus on timely access to healthcare services and support throughout their treatment.


Responsibilities:

  • Overseeing SIL activities and conducting SIL Assessments for the Aussie Life Care.
  • Coordinate care for patients, including scheduling appointments and arranging for diagnostic tests.
  • Communicate with healthcare providers to ensure timely followup and coordination of care.
  • Educate patients and families on healthcare services and treatment plans, including information and referrals to community resources.
  • Participate in quality improvement activities to improve patient care.
  • Working in partnership with a diverse group of professionals to deliver seamless and wellrounded care to clients.
  • Maintaining accurate and uptodate client records and reporting any changes or concerns to relevant parties
  • Participating in continuing education and training opportunities to maintain knowledge and skills.

Requirements:

  • Minimum of 2 years of experience in a healthcare setting
  • Knowledge of healthcare services and medical terminology
  • Excellent communication and interpersonal skills
  • Strong organizational skills and attention to detail
  • Ability to work in a fastpaced environment
  • Proficient in Microsoft Office and electronic medical record systems
  • Ability to work independently and as part of a team
  • Understanding of relevant laws, regulations, and community resources is desirable

Salary:
$ $38.00 per hour

Expected hours: 38 per week
