Overview

We have a fantastic opportunity for multiple Direct Care Workers to join our dedicated Community Health team in providing direct client service to individuals who are aged, frail, or disabled and want to continue living in their own home with some support.

The Direct Care Worker (Community) reports to the Community Care Coordinator and collaborates closely with Community Health professionals as part of a multi-disciplinary team.

We are seeking candidates with:

  • a demonstrated ability to work effectively within a team environment;
  • strong interpersonal and communication skills;
  • the capacity to engage with individuals from diverse cultures, backgrounds, and situations;
  • Demonstrated ability to meet deadlines and timeframes;
  • Capacity to offer assistance and support to colleagues;
  • Ability to exercise discretion and uphold strict confidentiality;
  • Capability to carry out general tasks under supervision.

Some exposure to working with elderly, frail, or disabled individuals in a community or hospital setting is desirable.
