Our aim is to help people live independently at home and stay connected with local communities. The Out of Hospital Care (OHC) program collaborates with local Health Districts to offer supports & services to people coming home after a hospital stay, ensuring a safe transition and ongoing connection.

About the Role - Care Manager - Out of Hospital Care

We are excited about a fantastic opportunity for a skilled and motivated Care Manager to join our OHC team in a full-time permanent role. In this position, you will liaise with external service providers and the OHC team to link clients to supports & services for a secure return home post-hospitalization. Your responsibilities include:

  • Reviewing referral and intake information and conducting assessments of OHC clients promptly either face to face or via phone within set time frames and KPIs
  • Developing person-centered care plans, recognizing care needs, outlining care plans, elucidating rights & responsibilities, and collaborating with clients to establish expectations
  • Identifying essential referral networks and accessing local resources aligning with OHC client care plans and budgets

You should possess:

  • A diploma or degree in Health or Allied Health field or a minimum of 6 years' relevant experience in a health-related discipline
  • At least two years of experience in a case management or client management role
  • Evidence of full Covid-19 vaccinations
  • Effective communication, advocacy, and interpersonal skills
  • Demonstrated time management, prioritization, and budgeting skills
  • Proven experience in case management
  • Current Australian Driver's Licence and access to a reliable vehicle
  • Proficiency in the Microsoft Office suite and the aptitude to acquire skills for in-house client database systems
