Position Summary

Health Independence Program Care Coordinator

  • Part time, 0.6 EFT available to cover LSL
  • Day work only
  • Work in a team that focuses on assisting clients manage chronic illnesses in a proactive and planned way
  • Based in Gippsland, Latrobe Regional Health are looking for a Care Coordinator to join our Health Independence Program on a
    part
    time,
    fixed term basis.
  • Our Health Independence Program helps clients manage their chronic disease in a proactive and planned model.
  • The Care Coordinator is responsible for identifying clients and working with them to coordinate the management of their chronic conditions. Working as part of a multidisciplinary team, along with external health care providers to coordinate chronic disease management.
