About Us:

Programmed Care provides care, support and home services to clientss throughout australia across a variety of Programs. We provide care and support services to Veterans Health, Aged Care and Community Nursing Industries.

Position Purpose:


The Care Consultant is responsible for the overall day-to-day provision of services with in the Vets and Aged HCP Progam.

As a Programmed Care Consultant you will work closely with the Care Coordination manager to provide high quality customer service to support our aged care clients receiving direct care in their homes, and support accessing the community.

You will work directly with the Programmed Care Team to develop relationships with clients and families to deliver strong supply teams for all clients.


Key Requirements:

  • Proven experience in a similar arole withing the community, aged or disability care environment
  • High level of communication skills that display a commitment to care and empathy for clients and staff
  • In conjuction with Care Coordination Manager, support clients wirh establishing and maintaining HCP budget
  • Ensure client services are managed as per HCP Service Agreement
  • Strong organisatiaonal and time management skills
  • Strong attention to detail
  • Strong background in client relationship management

Day to Day Duties:

  • Provide responsive, individualised services and support or clients and family
  • Network and Liaise with clients, organisations and the community
  • Support our clients to achieve their goals
