We are seeking a Community Connector to work on the Connecting Communities to Care project in the Glen Eira Council region.
The Connecting Communities to Care project sees to bring together clinical and social initiatives for older Australians by utilising existing resources and activating local, diverse, community members to work together.
About you:
- Bachelor's degree in health and/or social sciences, or equivalent qualifications
- Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)
- Experience of data collection and providing monitoring information to assess the impact of services
- Experience of partnership/collaborative working and of building relationships across a variety of organisations
- Knowledge of the personalised care approach
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
- Knowledge of motivational coaching and interview skills
- Importantly, you will mirror our values of Be Kind; Listen; Be Curious; Always safe; and Always together in all aspects of your work._
