What You Will Be Doing
Peninsula Health is committed to the implementation of Advance Care Planning (ACP) with a particular focus on adults with chronic complex disease, life-threatening illnesses and the frail aged.
ACP facilitates an individual to appoint their medical treatment decision-maker, appointed support person and to document their care preferences in a legitimate format known as an Advance Care Directive or Instructional Directive.
The Advanced Care Planning Clinician works as part of a team who are responsible for providing ACP support across PH and in the community.
What You Need
We anticipate you will have:
- Allied Health Practitioner with current registration with Australian Health Practitioner Regulation Agency (APHRA) or eligible for registration with the relevant professional organisation (if Social Work).
- General registration as a Registered Nurse with Australian Health Practitioner Registration Agency
- Sound understanding of Advance Care Planning aims and underlying principles.
- Demonstrated commitment to person centred care and patient advocacy.
- Workplace experience supporting and mentoring colleagues / staff.
- Willingness to learn, maintain and advance knowledge relevant to the position.
- Commitment to work as part of a specialised team to achieve the ACP Program aims.
- Ability to supervise undergraduate students.
- Current Victorian Driver's Licence.
- Aged Care Police Check.
- Working with Children Check.
- Highly developed communication and interpersonal skills and demonstrated ability to work collaboratively within a multidisciplinary team.
