We have an exciting

Full-time Community Support Worker

( Mental Health) with a Max Term until December 2023, opportunity available in our HASI HUB.
***The Housing and Accommodation Support Initiative (HASI) is a state wide program that supports people with a severe mental illness to live and recover in the community in the way that they want to.

Services delivered under HASI encompass a wide range of activities that build independence in daily life.

Supports can include recovery planning, support to live independently, assistance to manage a tenancy, assistance to manage health and emotional wellbeing, as well as support to get involved in your local community.

These positions involve shift work, and you will need to be available to work flexible shifts that may include a combination of mornings, evenings, and weekends.


Based in

North Ryde

, you will report to the Team Leader in our HASI Team.

Key responsibilities will include

  • Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
  • Regular reviews of progress.
  • Collaboration with Community Mental health staff around clinical goals set for customer
  • Complete comprehensive risk management plans for customer
  • Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required
  • Supporting customers to explore their practical and complex needs
  • Keeping customer records up to date on a daily basis using on line systems
  • Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
  • Seek out innovative ways to enhance the customer journey

You'll also need

  • Technical literacy with proven ability to use Microsoft Office and systems for reporting
  • Minimum Cert IV in Mental Health or Disability or equivalent industry experience
  • Availability to work across a rotating roster
  • Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
  • Willingness to obtain an Australian Federal Police Check, WWCC and first aid certificate that meet New Horizons employment criteria prior to commencing with us
