Job Description

The Senior Practitioner - Access and Homeless Support is responsible for the provision of support, advice and expertise to the Access Intake and Assessment Specialist Team working in partnership with the Access Operational Manager, the Senior Practitioner supports the day to day functions of the Access team, supporting the achievement of client's goals and housing outcomes through the application of strength based, culturally responsive, trauma informed, client focused intakes, assessments involving both brief and crisis intervention work practices. Support the Access team Outreach roster across various locations, including the provision of mobile services, to ensure clients receive support in a timely manner, monitoring incoming client referrals, client intake and assessments and completing client file audits to ensure best practice is adhered to. The Senior Practitioner will also liaise with other Senior Practitioners/Team Leaders across the Alliance to ensure clients requiring specialized response are identified and supported accordingly. The Senior Practitioner will provide ongoing coaching, mentoring and development of staff/students and volunteers. The Senior Practitioner will also fulfil other duties to support programs as required.

The Toward Home Access Senior Practitioner reports to the Access / Homeless Support Operations Manager and works closely with all Toward Home service streams and external partners.

The duties of this role include;

  • Support the responsive engagement with clients accessing the LC Toward Home homelessness services
  • Supporting developed case plans through VI SPDAT and client interaction.
  • Assist the team to develop consistent and client centered responses to families and individuals to address presenting issues impacting on secure housing, such as mental illness, substances use, physical health and life skills.
  • In collaboration with the Operational Manager develop strategies to ensure customers are successfully transitioned and integrated into the service
  • Support teams with your comprehensive understanding of the sector and the challenges that are faced by people who are experiencing homelessness
