About the role

The Team Leader, Birthing in Our Community (BiOC), will support the Manager in the effective implementation of the Jajumbora BiOC Model of Care by managing and coordinating the Family Support Workers (FSWs) within the Hub. This hands-on role involves direct engagement with staff and clients, ensuring that comprehensive and culturally informed maternal and child health services are provided.

Duties

  • Establish and maintain regular reporting activities related to client record systems.

  • Conduct regular audits of Family Support Workers' (FSWs) caseloads.

  • Facilitate high-risk case conferences with FSWs.

  • Lead incident debriefing processes.

  • Provide monthly supervision sessions with FSWs.

  • Support the team in working confidently within the BiOC Model of Care.

  • Offer ongoing education and support to ensure compliance with organizational policies and procedures.

  • Develop individual Professional Learning Plans for team members, including clear goals and actions.

  • Lead targeted coaching to enhance team performance.

  • Enhance communication within the team to promote ATSICHS vision and core values.

  • Foster a positive team culture through targeted reinforcement and praise.

  • Ensure all scheduled tasks are completed within specified action day.

  • Support and manage a continuous improvement model to enhance service delivery and staff practices.

Required

  • Diploma qualification in Social Work / Human Services / Psychology / Sociology / Criminology / Counselling or relevant field and/or five (5) years’ experiences in relevant human service field e.g., Child Protection, Family Support, Youth Support.

  • Ability to work in a performance driven environment and meet month-to-month program targets/ performance outcomes.

  • Demonstrated experience in overseeing and coordinating case management processes and procedures. 

  • Ability to lead, guide, and manage people including the implementation of cultural change and change management opportunities.

  • Ability to support and manage a continuous improvement model to further improve the service model and practice application of front-line staff.

  • National Police Certificate.

  • Working with Children’s Card (Blue Card) or be eligible to apply
  • Driver's License.

Preferred

  • Knowledge or certification in maternal and child health, particularly in culturally specific contexts.

  • Experience in designing and implementing innovative health and community service program.
